Volunteer Forms

PA State Clearances

MinistrySafe is sponsored by our insurance company to provide greater consistency of training and the ability to better track background checks.

It has been part of our Safe Sanctuaries policy that all church staff and children & youth volunteers are required to take Safe Sanctuaries training when they first start in ministry and every five years thereafter. A reminder that starting January 2020 this course will ONLY be offered online through MinistrySafe.

Procedures for obtaining clearances themselves have not changed and they can be accessed through the Susquehanna Conference Safe Sanctuaries web page. Please contact safesanctuaries@susumc.org with any questions.

Note: For the safety of our campers, you will not be able to volunteer at our camp and retreat centers if you do not have all 3 clearances.

If you have not completed the required clearances within the last 5 years follow these instructions: Procedures for Volunteer Clearances

If you are unsure if your clearances are still up-to-date you may contact us at 717.766.7395 or email camps@susumc.org.

PA State Police Criminal History Report: Done online, usually instantaneous, though it can be pending if the system is busy.

PA Child Abuse Clearance: done online, can take up to 14 days to receive email with instructions for printing certificate

FBI Criminal History Report: can register online, but must go to site locations to submit fingerprints.  Can take up to 2 weeks to obtain results.

Volunteer Affidavit: can only be used for those who have continuously lived in the state of Pennsylvania for 10 years.  Please note you must have this affidavit notarized for it to be complete.

First Time Volunteers :

Please complete the following paperwork and submit it to the camping office at least 2 weeks prior to serving.
Volunteer Application & Certification
Volunteer Disclosure & Release Form
Adult Medical Form (volunteers under 18 complete camp registration form)

Returning Volunteers :

Please complete the following annual paperwork and submit it to the camping office at least 2 weeks prior to serving.
Volunteer Medical Form

Counselors

All assistant counselors must register for their event through CampBrain.

New Assistant Counselor:

If you are a new Assistant Counselor, please fill out all First Time Volunteers paperwork (scroll up). Since you are under age 18, we do not need the PA  State Clearances. The Assistant Counselor Fee is 50% of the Early Bird Cost for the week of camp you are volunteering for. Assistant Counselors are required to submit a paper Camp Registration Form to the Camping Office. Please note:  Before you submit forms, please be sure you have been accepted by a summer camp Dean. Submitting a form does not guarantee volunteer acceptance. If you need help connecting with a Dean, please contact the Center Director

Returning Assistant Counselors:

If you are a returning Assistant Counselor, please fill out the Volunteer Disclosure Form and Covenant and the Volunteer Medical Form.
